Requirements:
<br>

1. Minimum of high school diploma or GED
<br>
2. Minimum of 2 years experience working with persons with disabilities
<br>
3. Valid Arizona Driver’s License
<br>
4. Ability to demonstrate good observation skills, judgment and interest in people with disabilities
<br>
5. Good verbal and written communication skills; bilingual preferred
<br>
6. Documentation of Class I Fingerprint Clearance card must be provided within 60 days
<br>
7. Must provide current certification in CPR, First Aid, CIT I, CIT II, and Article IX within 60 days of employment
<br>
8. Participate and provide documentation for a minimum of 12 continuing education hours annually.

Duties:
<br>

<br>
1. Responsible to Adult Services Manager or Director of Human Services
<br>
2. Responsible for all documentation as required by VR, DDD, etc.
<br>
3. Responsible for assuring that the needs of participants involved in ISE and ESA contracts with DDD are meeting their stated objectives
<br>
4. Assist with accountability through participant matrix, individual participant books, attendance records, etc. for participants
<br>
5. Assist in the development and implementation of schedules and opportunities for adults to gain independence, self help and social skills and meet their objectives in their individualized support plan
<br>
6. Assist in assuring that all contract and license requirements (VR, DES/DDD, DES/Childcare Licensure, etc.) are being met including; writing objectives, teaching and behavior plans, data collection, monthly reports, incident and physical management reporting, etc.
<br>
7. Model and participate fully in all Arts for All activities.
<br>
8. Assist in dispensing medication
