The NURSE PRACTIONER is responsible for the overall patient care management process. An advocate for the patients, the Nurse Practitioner will provide total care for the patients in collaboration with the physician of record and AzPCP team. The NP will communicate and collaboratively manage the patients’ care across all care providers, family, and caregivers. The NP will ensure that timely, patient-centered, appropriate evidence-based medicine is provided. The Nurse Practitioner will provide direct patient care in our care centers as well as with patients telephonically with occasional home visits as needed.
• Applicants must hold a current Arizona NP license in good standing and current ACLS certification
• Minimum of 3 yrs. experience as an NP in a direct clinical setting; experience in Internal Medicine or Family Practice preferred
• Prior experience working with seniors and chronic disease management
• Excellent assessment skills required; able to problem-solve with an awareness of level of care, intensity of service & severity of illness criteria
• Flexibility in their work environment as this position will initially begin as telephonic management with occasional home care visits and transition into our care clinics
• Willingness to help address the wrap-around issues of chronic disease management: patient education, psycho-social issues, family/care-provider issues, etc.
• Knowledge of interviewing practices and techniques used in order to gather information.
• Strong interpersonal skills with the ability to build relationships with providers and members.
• Excellent verbal, written communication and listening skills both in-person and on the phone required.
• Knowledge of and comfort using computer based applications such as MS Office and electronic medical record systems.If you care about your patients, can think outside the box and have a background in Internal medicine, Family Medicine, or Geriatrics, we want to speak to you.
